RVA Healthy Hearts HUB

 

The RVA Healthy Hearts Hub is a community-centered expansion of the RVA Healthy Hearts Initiative. We empower Richmond residents to improve their heart health through education, support, and practical tools to manage high blood pressure and prevent cardiovascular disease.


Participant Benefits (All FREE)

  • Essential Gear: Free blood pressure monitor and a daily health tracker.
  • Expert Coaching: Monthly wellness sessions and at least two private wellness check-ins.
  • Community Support: Access to "Office Hours" on the 1st & 3rd Monday of each month (5:45 PM – 6:15 PM) and referrals to local resources like fitness groups.

To be eligible for the program, participants must meet the following:

  • Age: 18 years or older.
  • Diagnosis: Diagnosed with high blood pressure/hypertension or taking blood pressure medication.
  • Medical Safety: Must not have had a cardiac event (heart attack/stroke) in the last 12 months; must not have an irregular heartbeat (A-Fib) or risk of lymphedema.


 We offer seasonal cohorts to ensure you have a dedicated community of peers:

  • Summer Program: Enrollment is OPEN NOW throughout the month of June. Register anytime in June to secure your spot for the summer journey! Click Here
  • Fall Program: Enrollment will begin in September.


We are looking for residents ready to take charge of their health. Participants commit to:

  •  Monthly educational sessions delivered via YouTube links. 
  • Monitoring and tracking blood pressure daily.
  • Attending wellness check-ins and engaging with the community hub at least 2 times per month.


We meet you where you are! On the 1st and 3rd Monday of the month, you can connect with your coach via video link, a quick phone chat, or in-person at our designated Richmond location. It’s a great time to review your numbers and troubleshoot challenges. 


Each month, you’ll receive a YouTube link. You can watch the training on your own time, in your pajamas, or on your lunch break—and you can re-watch it whenever you need a refresher. 


 Yes. Your blood pressure monitor, health tracker, and all coaching sessions are provided at no cost through the Virginia Healthy Hearts Initiative. 


 Yes. Once you finish the program, the monitor and tracker are yours to keep so you can continue staying on top of your heart health. 


 It’s a safety and accuracy thing! Home monitors can sometimes struggle to give an accurate reading with an irregular heartbeat. We want to ensure you get the most precise data possible, so we can refer you to programs better suited for those specific health needs. 


Yes. Your tracking data is used specifically by your coach to provide you with personalized wellness support and is handled with strict confidentiality. 


To see real results, we ask that you self-monitor your blood pressure at least 4 times per week and document your numbers using one of these three methods:

  1. Electronic Tracker (Preferred): Download and use our digital RVA Healthy Hearts Hub Official tracker.
  2. My Daily Health Tracker Booklet: Hand-write your numbers in your provided booklet.
  3. PDF Tracker Form: Download, print, and fill out the form manually.


To provide you with the best coaching, trackers are due twice a month during our wellness check-ins:

  • Due Date 1: The 1st Monday of every month.
  • Due Date 2: The 3rd Monday of every month.

Submission Methods:

  • Electronic: Email your completed form to drblackwellconsult@gmail.com 
  • Booklet: Take a clear photo of your tracking pages (make sure your name is visible) and text it to 804-586-5679 or email it to drblackwellconsult@gmail.com.
  • PDF: Scan and email your completed form to drblackwellconsult@gmail.com 


Not necessarily! The program is open to residents of the Greater Richmond area and the Petersburg/Tri-Cities area. 


 Once enrolled, you can pick up your monitor in person on the 1st or 3rd Monday of the month at our Richmond location. Pickup takes place during our wellness check-in window from 5:45 PM – 6:15 PM. At pick up, you will be asked to complete the Consent Form before receiving the monitor.
